LPN Remote Patient Monitor (SPANISH FLUENT)

Connect RPM is a fast growing healthcare company that provides Remote Patient Monitoring services to chronic care patients all over the United States. Our mission is to provide better care and outcomes to high risk patients. We are seeking a full-time LPN to work from their home office while complying strictly with HIPAA privacy laws. Essential Functions • LPN only please, we cannot legally hire MA's for this position. • MUST BE FLUENT IN SPANISH • Must be in a Nursing Compact state. • Remotely monitors the patient's blood pressure, glucose levels, weight, oxygen, etc. • Training patients on the use of our blood pressure cuffs, glucose meters, pulse- oximeters, etc. • Communicate with patients. • Call out critical results. • Abide by the plan of care and orders of the practice. Competencies Required to Perform the Position • Maintain a quiet and focused work environment from home. • Understand health care goals associated with chronic disease management provided by the practice. • Be legally able to work in the United States. • Ability to provide prevention and intervention for multiple disease conditions through motivational coaching. • Must be proficient in data entry. • Must maintain a friendly and happy patient attitude. • Experience using Electronic Health Records. Work Environment This job operates in the comfort of your home setting and routinely uses standard office equipment such as computers and headsets for internet calls. You will need to have wired, high-speed internet, a private space free from noise, and an organized workspace for your computer. Position Type and Expected Hours of Work This is a full-time position, 8 hours daily, Monday - Friday Required Education and Experience • LPN only please. (We cannot legally hire you for this position if you are an MA) • A minimum of 1 year clinical experience in Primary Care, Cardiology, Renal, Endocrine med/surg, home health care, geriatric nursing, or similar. • Hands-on experience with Electronic Medical Records as well as knowledge of Windows desktop, Excel, and applications is strongly preferred • Able to exercise initiative, judgment, organization, problem-solving, and decision-making skills is a must to be successful in this role • Skilled in developing and maintaining effective relationships with team members and clients • Capable of organizing work to achieve daily/weekly goals and objectives • Excellent verbal, written and listening skills are a must. Bilingual in English and Spanish is a plus.
